    before presenting for your appointment.

•    Obtain necessary PCP or HMO referrals.

•    Notify us of plan or benefit changes.

•    Present new insurance cards.

•    Notify us of address/phone/email changes.


If you are unable to keep an appointment for any reason, including lack of insurance coverage, please
reschedule at least 24 hours in advance to avoid a $25.00 missed appointment fee.

Rescheduling, due to lack of insurance coverage,
